Community Capacity Building Funding
Health-Related Social Needs
Starting in 2024, Coordinated Care Organizations (CCOs), providers, community organizations and others will launch a set of non-medical services not typically offered through Medicaid to Oregon Health Plan members as part of Oregon’s Health-Related Social Needs (HRSN) Initiative. HRSN services will include housing-related supports, nutrition services, and climate devices for qualifying individuals experiencing a life transition.
Community Capacity Building Funding
Oregon is providing HRSN Community Capacity Building Funding to support organizations interested in becoming HRSN providers. The Oregon Health Authority will distribute this funding regionally to CCOs including Trillium. Trillium and other CCOs will be responsible for reviewing applications, awarding funds, and monitoring use of the funds.
How to Apply
Thank you to everyone who submitted HRSN Community Capacity Building Funding applications. The application process closed May 31, 2024. Applicants will be notified of the status of their application no later than September 15, 2024.
Allowable Uses for Community Capacity Building Funding
Examples of allowable Community Capacity Building Funding categories and services include:
- Technology (procure IT infrastructure/data platforms, develop an HRSN eligibility and services screening tool)
- Development of business or operational practices (develop related policies and procedures, purchase a commercial refrigerator to provide additional medically tailored meals to qualifying members)
- Workforce development (cost to recruit, hire and train new staff, salary and fringe for staff that will work directly on HRSN for up to 18 months)
- Outreach, education and convening (translate materials, plan for and facilitate community-based outreach events to support awareness of HRSN services)
