Community Health Improvement Plan

Trillium is committed to working with members and partners to address unique community health needs and priorities. The Community Health Improvement Plan (CHP) is specific to each region and details community health priority areas, goals, and strategies to be implemented within the region in collaboration with community partners.

Trillium community investments, including sponsorships, Community Benefit Initiatives (CBI) and Supporting Health for All through REinvestment (SHARE), are aligned with the CHP priorities.

The Community Advisory Council, including Trillium members and community partners, in each region is responsible for approving, monitoring, and guiding funding decisions for the CHP in each region.

Lane County CHP

Live Healthy Lane is a community-based effort to improve the health and well-being of those who live, learn, work, and play in the Lane County Region. Live Healthy Lane unites community partners, local public health organizations, CCOs within the service region, and healthcare providers to reduce health disparities, promote health equity, and improve overall population health. Together we work on the Community Health Assessment (CHA), which explores and identifies health issues in Lane County.

The CHA creates an opportunity to improve the health of our community by informing the Regional Community Health Improvement Plan (CHP) (PDF).

The CHP is an action-oriented plan with goals to:

  1. Ensure incomes are sufficient to meet basic costs of living (i.e., housing, childcare, food, transportation, etc.)
  2. Establish community conditions that support behavioral health and physical well-being
  3. Address current historical injustices that produce disparities

Tri-County CHP

The Healthy Columbia Willamette Collaborative (HCWC) is a community-based partnership including local public health authorities, hospitals, health plans, and CCOs from Clackamas, Multnomah and Washington counties working to develop a Community Health Needs Assessment (CHA) every three years and support coordinated implementation of local CHPs.  

The 2025 – 2029 CHP is focused on:

  • Access to Equitable Care 
  • Prevention
  • Social Health Needs
