Supporting Health for All through Reinvestment (SHARE)

Through the SHARE Initiative, Trillium is proud to invest in projects and initiatives that address health inequities and social determinants of health and equity (SDOH-E) to improve health and wellness for Oregonians. These reinvestments align with our Community Health Improvement Plan (CHP) and support upstream, non-healthcare factors that impact health, such as housing, food, transportation, educational attainment and civic engagement.

Southwest 

  • Bushnell University
    Initiative for Accelerated Nursing: funding to recruit and train nurses who will be highly skilled in delivering culturally competent care and treating patients in community and integrated behavioral health settings.
  • Drawbridge Innovations
    Facilitate collaborative workshops with community organizations and collaborators to develop process for operationalization of Oregon’s Health-Related Social Needs housing benefit for qualifying Oregon Health Plan members.
  • Equitable Social Solutions
    Housing supports for 55 families, including rent, utilities, fees, deposits, education, and support services.
  • FOOD For Lane County
    Trillium Produce Plus & Trillium Veggie Rx: free, fresh produce in clinics and other settings that provide services to populations impacted by food insecurity, plus produce program for Trillium Start Smart for Your Baby® participants. 
  • FUSE (Frequent Users Systems Engagement)
    Help participants address barriers to housing; provide case management, and connect participants to community resources and supportive housing placements that help minimize utilization of crisis services.
  • Mobile Crisis Intervention Services (MCIS) Startup
    MCIS is focused on early intervention and crisis de-escalation, with a focus on diverting unnecessary trips to the emergency department, hospitalizations, child welfare involvement, juvenile justice or arrests, and providing services and including connections to housing, food, and more.
  • Oregon Health Care Interpreters Association
    Fund the recruitment, training, and credentialing of 30 new health care interpreters (HCIs) in high-demand languages to increase patient safety and improve health care outcomes for individuals who speak languages other than English.
  • St. Vincent DePaul
    Families Annex Kitchen: capital improvements to the kitchen that will serve families experiencing homelessness who reside at the Annex; First Place Day Care: accessible program designed to support families experiencing homelessness staying at the Annex with a therapeutic preschool, family resource referrals, family support activities, and community engagement; Pallet Shelters: providing emergency shelter with a capacity to house six families at a time. 
  • United Way
    Early Learning Hub: perinatal to age 5 Resource Navigation Support System (Pollywog system): Connect participants to services such as pregnancy support, childbirth education, lactation support, parenting education, early learning, and social emotional health supports.

Tri-County 

  • Cascade AIDS Project
    Esther’s Food Pantry: increase access to food and decrease food insecurity at this community food pantry.   
  • Central City Concern
    16th & Burnside: complete construction and renovations to this 74-bed, ADA accessible residential treatment program staffed with case managers who will support residents with obtaining stable housing, employment or education opportunities, and more.
  • Clackamas County Recovery Center
    Funding for 16-20 beds in Substance Use Disorder Treatment Recovery-Oriented Housing in this new facility.
  • Fora Health
    Construction of a 46-bed recovery housing project adjacent to the new Fora treatment center in Southeast Portland.
  • Greater New Hope Charities
    BIPOC Community Health Worker (CHW) training program for 75-80 people, which is free to BIPOC participants who are low-wage earners or unemployed. All facilitators and trainees identify as BIPOC.
  • Oregon Health Care Interpreters Association
    Fund the recruitment, training and credentialing of 100 new health care interpreters (HCIs) in high-demand languages to increase patient safety and improve health care outcomes for individuals who speak languages other than English.
  • Oregon Public Health Institute (OPHI)  
    Right from the Start (RFTS): support childcare providers, traditional health workers, and early life professionals in creating holistic healthy learning environments for children ages 0-6 in Portland’s African, African American, and Black communities.
  • Tri-County Behavioral Health Providers Association
    Certified Alcohol and Drug Counselor (CADC) Scholarship Funding Coordination: Support a loan fund that pays for student tuition and books including core classes for CADC and practicum. After completing 12-months of employment working for any participating TCBHA agency, the loan will be forgiven.

  • FUSE (Frequent Users Systems Engagement)
    Outreach to help participants address barriers to housing and connect to community resources, as well as case management support.
  • FOOD For Lane County
    Trillium Produce Plus & Veggie Rx: free, fresh produce in clinics and other settings that provide services to populations impacted by food insecurity, plus produce program for Trillium members with a health condition who will benefit from more fresh fruits and vegetables.  
  • Homes for Good
    Bridges on Broadway: fund the construction of the former Red Lion into 56 units of permanent supportive housing for people with high needs and experiencing chronic homelessness, plus additional capital expenses at select sites. 
  • Lane Stabilization Center
    Funding for staff who will plan and develop programs that combat health inequities and address social determinants of health; support for community engagement with individuals with lived experience; and holistic care with connection to long term supports.

Tri-County 

  • ​​Clackamas County Stabilization Center
    12-hour Stabilization Center: capital improvements for this facility which supports community members in their recovery journeys, including enabling access by members with disabilities.
  • IRCO (The Immigrant and Refugee Community Organization)
    CHW Training & Workforce Development: funding for the required 90-hour Community Health Worker (CHW) training for 15 participants recruited from IRCO and the community at large.
  • Samoa Pacific Development Corporation
    Pacific Islander Housing Assistance: funding for staff who will implement community education initiatives, workshops, and outreach campaigns and provide direct housing assistance.
  • Washington County Center for Addictions Triage and Treatment (CATT)
    Capital improvements for this facility which offers transition services, peer drop-in center, crisis services, co-located supports, care coordination, preventative services, and a culturally specific outpatient program supporting Latino/a community members.

Southwest

  • Burrito Brigade
    Emergency food assistance through funding burrito ingredients, plus foods like canned protein, peanut butter, and canned soup for the Little Free pantries and Waste to Taste.
  • Carry It Forward
    Help fund a mobile shower unit, ADA Shelter unit, kitchen unit, laundry unit, and a Personal Health Navigator for Tom Campbell unhoused camp.
  • CORE
    Fund staffing, renovation, transition costs, and furnishing during the move-in period at the Advocacy Center for at-risk young people, a safe space to access free Life Skills workshops, a Community Garden, resume and job support, harm reduction supplies, basic needs, and one-on-one case management.
  • Eugene YMCA
    Help fund the construction of the new Lane County YMCA, specifically the Rainbow Preschool Room, which serves pediatric clients.
  • FOOD For Lane County
    Supports the FOOD for Lane County Produce Plus program and expansion to additional sites serving BIPOC and LGBTQIA clients and promoting a food insecurity screening tool.
  • FUSE (Frequent Users Systems Engagement)
    Outreach to help participants address barriers to housing and connect to community resources, as well as case management support.
  • Square One Villages
    Capital construction costs for Peace Village Co-op which consists of 70 units of high-quality, energy-efficient, permanently affordable co-op housing for people with low incomes.

Tri-County

  • All: Ready Regional Kindergarten Readiness Network
    Support families to promote kindergarten readiness, with a specific focus on Black, Indigenous, and communities of color, including resources to support development and cultural wellness, needed diagnosis around a developmental or learning disability, and more.
  • Community Development Corporation of Rockwood
    Support for culturally specific food pantry and delivery program which provides meals to vulnerable families and funding for a bilingual, multi-cultural Community Health Worker to support houseless families/individuals living at Rockwood’s transitional housing shelter hotel.
  • Harmony Academy
    Outreach to youth and communities impacted by substance use disorder, especially youth who might have experienced barriers to accessing appropriate support for substance use and co-occurring disorders.
  • IRCO (The Immigrant and Refugee Community Organization)
    Construction of a wheelchair ramp to provide ADA access to the IRCO bazaar, a storefront for free items like clothes, furniture, and household and personal items.
  • Latino Network: La Plaza Esperanza
    Help fund a capital campaign to expand a community center providing Community Health Worker services.
  • Quest Center for Integrative Health
    Transitional housing for LGBTQIA+/African American, HIV positive clients as well as community-based health improvement interventions, including group meetings to address health concerns related to substance use disorders.
  • Raices de Bienstar
    Funding for Fomentando Fortalezas, a monthly program for Latine traditional health workers to join and practice culturally adapted self-care skills & exercises in Spanish.
