Apply Now: Health-Related Social Needs Community Capacity Building Funding: March 1, 2024

Trillium Community Health Plan (Trillium) is now accepting applications for Health-Related Social Needs (HRSN) Community Capacity Building Funding (CCBF). In 2024, there is a total amount of $1,398,085 in CCBF grant funds available for Trillium's Tri-County service area and a total amount of $1,339,151 in CCBF grant funds available for Trillium's Southwest service area.
If your organization would like to provide HRSN services, which include housing-related supports, nutrition services, and climate devices for qualifying individuals experiencing a life transition, you may be eligible to apply for Community Capacity Building Funding.
How to Apply
Organizations that would like to apply for HRSN Community Capacity Building Funding must take the following steps:
- Complete Trillium's online application form.
Note: If you'd like to view the application in PDF for reference only, please refer to the application in PDF format (PDF), Solicitud (PDF). However, you must submit your application using the online application form to receive funding consideration. - Complete the budget funding budget funding request (PDF), Solicitud de Financiación del Presupuesto (PDF) and submit it as an attachment when you fill out the online application.
Applications will be scored using a standardized Community Capacity Building Funding Scoring Rubric (PDF), Rúbrica de Puntuación para la Financiación del Desarrollo de Capacidades Comunitarias (PDF).
Trillium coordinates benefits for residents of Lane, Western Linn (Harrisburg), Western Douglas (Reedsport), Multnomah, Washington, and Clackamas Counties. Organizations may apply to more than one CCO to meet the needs of their service area. However, funding requests must be non-duplicative.
Applications will be accepted starting March 1, 2024 and close May 31, 2024.
