Wellcare By Trillium Advantage Invests more than $1 Million in Funding for Mobile Medical Outreach in Lane County

Partnerships Aim to Bring Healthcare Services to Dual Special Needs Plan Members
Wellcare By Trillium Advantage (Wellcare) is proud to invest more than $1 million in funding for mobile medical outreach in Lane County. Through partnerships with Cascade Health,† Center for Family Development,† G Street Integrated Health,† Northwest Medical Homes† and Oregon Integrated Health,† Wellcare’s investment will bring physical and behavioral healthcare services to Dual Special Needs Plan (DSNP) members in their homes. DSNP members qualify for both Medicare and Oregon Health Plan/Medicaid insurance coverage. The five healthcare organizations receiving Wellcare funding are investing in vehicles and mobile medical vans that allow them to travel to DSNP members’ homes to provide services.
“Wellcare is committed to partnering with providers and local organizations to increase access to whole-health solutions and services for our Lane County members who live in both urban and rural communities,” said Suellen Narducci, Trillium Community Health Plan Chief Operating Officer. “This means addressing the social determinants of health, removing barriers to care such as transportation, and meeting our members where they are to provide clinical care and connect them to additional social and community resources.”
G Street Integrated Health is serving DSNP members in the Eugene/Springfield area, and Oregon Integrated Health is serving members in the Florence/Reedsport area. Both organizations will offer mobile medical vans fully equipped for patient care.
"Our mobile medical van plays a crucial role in improving access to healthcare services in Lane County. By bringing medical care directly to the community, we can help individuals who might have transportation challenges or face other barriers to healthcare access. We can perform physical exams as well as basic health screenings like blood pressure checks, blood draws, and even EKGs to help identify potential health issues early on and allow for timely interventions. Our mobile medical van is a valuable resource for improving health outcomes and reducing health disparities in our community," said Hollie Imler, LPN, Director of Clinical Services, G Street Integrated Health.
“Our providers are passionate about ensuring our patients have access to the whole-health services they need, whether that be primary care, counseling and mental health services, physical therapy, or health and wellness coaching,” said John Howell, Doctor of Chiropractic, Owner and Administrator at Oregon Integrated Health. “This mobile medical outreach service represents a true team effort on behalf of our entire staff. We’re excited to collaborate with Wellcare to reach people where they live, create a personal connection, deliver needed care, and help build healthy communities.”
Northwest Medical Homes has partnered with the Center for Family Development to provide a full array of services to their DSNP population, including home visits and case management.
“Many of our patients have social or medical needs that can make it difficult to travel to a clinic,” said Jordan Moon, PA-C, Northwest Medical Homes. “We sincerely appreciate this partnership with Wellcare that enables us to visit patients in their homes, understand their healthcare needs, identify any barriers to care, develop solutions, and create a cadence for regular care going forward.”
Cascade Health providers are meeting with members in their homes to conduct needs assessments, evaluate patients for non-emergent medical concerns, perform mental health evaluations, and connect members to any additional resources they may need.
