Resources for Health-Related Social Needs Providers
Is your organization interested in becoming a Trillium Health-Related Social Needs provider?
Starting in March 2024, Coordinated Care Organizations (CCOs), providers, community organizations and others launched a set of non-medical services not typically offered through Medicaid to Oregon Health Plan members as part of Oregon’s Health-Related Social Needs (HRSN) Initiative.
HRSN services include:
- Climate-related supports - launched March 1, 2024
- Housing-related supports for members who are at risk of homelessness* - launching November 1, 2024
- Nutrition services - launching January 1, 2025
* The State is still determining when housing services will go live for other eligible members.
