Trillium's 2025 CCBF Launching April 1, 2025

Community Capacity Building Funding Kickoff Call and Info Sessions

Trillium is excited to announce the 2025 Community Capacity Building Funding (CCBF) opportunity!
 
CCBF provides grant funding to support community-based organizations in becoming Medicaid Health-Related Social Needs (HRSN) providers. HRSN utilizes Medicaid funds to provide housing support, nutritional assistance, climate interventions, and outreach and engagement. CCBF grants may be used to fund technology, development of operational or business practices, HRSN workforce development, and outreach, education, and convening related to HRSN services. Applications open April 1, 2025, and close May 30, 2025, at midnight. The Trillium website will be updated with a live application link on April 1st.

Funding Overview:
The Oregon Health Authority has authorized Trillium to distribute $4.1 million total across its regions:

  • Tri-County (Multnomah, Clackamas, Washington): $2,207,184.11
  • Southwest (Lane, Douglas, Linn): $1,902,253.93

Office Hours: unstructured time for potential applicants to ask specific questions and get clarification.
