Trillium's CCBF Grant Application is Live Now!

Thank you for attending Trillium Community Health Plan’s recent webinar for the 2025 Community Capacity Building Funding (CCBF) grant cycle. Your participation is vital as we work together to address Health-Related Social Needs (HRSN) services across Oregon. Together, we can make a significant impact on the health and well-being of our communities.

CCBF have been instrumental in growing Oregon's provider network for HRSN services. In 2024, CCBF investments enabled over 50 community-based organizations (PDF) to enhance their capacity to deliver essential services. These investments have fostered stronger community partnerships and increased access to care for vulnerable populations.
 
Trillium 2025 CCBF:

As a reminder, the 2025 CCBF application is now live and will close on May 30, 2025 at midnight.
 
This year Trillium is distributing a total of $4.1 million in funding:

  • Tri-County (Multnomah, Clackamas, Washington): $2,207,184.111
  • Southwest (Lane, Douglas, Linn): $1,902,253.93

Trillium's CCBF aims to support organizations interested in becoming HRSN providers. The funding can be used for various purposes, including technology, development of operational or business practices, HRSN workforce development, and outreach, education, and convening. For detailed information on allowable uses of CCBF funds, please refer to the CCBF grant application.
 
Trillium is prioritizing applications from community-based organizations that demonstrate funding will increase capacity to accept external referrals for HRSN services for rent assistance, tenancy support and outreach and engagement, and that intend to provide these direct services, especially those serving culturally specific populations and expanding capacity, in Multnomah County, Lane County, Reedsport, and Harrisburg.
