Care Coordination and Case Management

What Is Care Coordination and Case Management? 

Trillium is here to help you meet your healthcare goals. We work to coordinate care based on your unique needs. We work with you, your providers, and anyone else that you designate, to create a comprehensive pathway to wellness and prevention. 

We use a coordinated approach, connecting you with community resources and our care team to make sure your needs are met. This approach helps you navigate the healthcare system to achieve whole person wellness. This includes physical health, behavioral health treatment, and oral/dental health. We also assist in removing barriers. 

You and your care team will participate in creating goals to improve your health and wellbeing. This care team may include:

  • your doctor 
  • your therapist
  • your family
  • anyone else important to your health outcomes 

When you are in a Care Coordination or Case Management Program, you will have an assigned care manager. Your care manager will work with you to develop a care plan. This plan will have activities to help meet your needs. Your care plan will be updated at least every year. Your care plan may be updated sooner, as needed, when you achieve your goals or your needs change. 

You can ask for a care coordinator at any time. Call Member Services at 1-877-600-5472 (TTY: 711). Ask to speak to a care coordinator. 

We may also contact you if your provider or another community resource asks us to help.
