Our Care Management Program

Welcome to the Trillium Care Management program. This is for pregnant parents who need support. Trillium wants to make sure you get the care you need to have a healthy pregnancy and a healthy baby.

Trillium's Care Management team will work with you to answer your questions. Each Trillium member who engages with Start Smart for Your Baby® will have a dedicated RN Case Manager (CM) for support. Your CM will outreach and provide education throughout your pregnancy. As your team, we can help schedule appointments, provide transportation assistance, and find resources in your community. We can also help with other behavioral and social services. 

We can help find services in the community that will help you improve your health. We will work with you to:

  • Identify and address your basic needs
  • Help you get the services and support you need within your community
  • Teach you and your family how to manage your own care and resources
  • Find a pediatrician in-network before your baby's arrival for ease of care coordination

Our professional and diverse team has prenatal, labor and delivery, post-delivery, newborn nursery, and NICU experience. We are here to assist you through every step of your pregnancy.
